| 현병력 | 2018년 GB polyp으로 cholecystectomy 받은 후 정확히 알 수 없는 이유로 bile duct resection, Roux-en-Y hepaticojejunostomy 시행받은 과거력이 있는 자로 내원 3시간 전부터 발생한 발열, 의식저하로 내원하였다. |

| 치료 및 임상경과 | Hepaticojejunostomy anastomotic stricture에 의한 acute cholangitis, septic shock으로 내원했던 자로 PTBD 후 증상 호전되었고 근본적 치료 위해 short-type single balloon enteroscopy-assisted ERCP를 시행하여 covered metal stent와 plastic stent를 삽입하였음. 시술 후 asymptomatic pneumoperitoneum이 있었고, 대증적 치료후 퇴원함. 2주후 외래에서 시행한 X ray상 metal stent internal migration이 발생하여 첫번째 시술 한달뒤 재입원하여 스텐트 제거 위해 다시 short type single balloon enteroscopy를 시행하였으나 approach fail하였음. rescue로 conventional long type single balloon enteroscopy를 시도하였고, approach에 성공하여 스텐트를 제거하였음. |
