AP CT(2021/3/23): About 2.1cm length, pancreaticolith in head portion. - diffuse upstream duct dilatation (about 8mm in size) Another tiny pancreatic calcifications in tail portion. About 1.9cm sized GB stone in neck. |

| 치료 및 임상경과 | Intraductal stone in Main P-duct c panrenchymal atrophy, GB stone. 치료위해 내원한 분으로 3/23일 1차 EGD+EUS 시행함 ->Pancreas: huge intra ductal hyperechoic lesion on head portion with diffuse dilatation of P-duct(>1cm in M. diameter), atrophied parenchyma GB: dilatated with huge hyperechoic lesion inside 3/29일 2차 f/u EUS 시행함 ->Pancreas duct on body was punctured with 19G needle(0lympus) under EUS guidance. Pancreas juice was confirmed by suction. Contrast dye was injected. After advancing guide wire(0.035, Visiglide) to head portion with coiling. Cystotome was used for dilatation of fistula tract. however, deployment of GIOBOR stent failed d/t dislogement of guide wire. -> ESWL - Pancreas intraductal stone in head portion 제거위해 비뇨기과 협진 2> IV ceftriaxone 3.29- 4/1일 ESWL 시행함. ->Rt. L1 transverse process에 걸쳐서 Pancreatic stone (21mm) 관찰됩니다 ->ESWL 2,500회 시행함 ( power: 13 ) 4/5일 EGD+EUS f/u 시행함 ->Fistula dilatation was done with Soehendra retriever(6 and 8.5 Fr) step by step. Finally, SEMS(GIOBOR, partially un-covered, 80mm, Taewoong) was deployed in Scope deployment method. -> 시술후 부작용 없음. 컨디션 좋음. v/s stable, Lab 호전 -> 4/7일 퇴원 |
